Student Health Record and Immunization Information
Welcome to The College of Health Professions, School of Occupational Therapy at Pacific University. You will be required to submit health records and immunization documentation to the University and School of OT before you begin the program in late August. It is very important that you follow the instructions below to assure that you will be prepared to fulfill these requirements. Students not in compliance with these requirements will have to postpone their fieldwork rotations, disrupt their progress in courses, or both.
Some of the health record information and immunizations may take up to nine months to complete; therefore, getting started immediately will support your compliance with the programs requirements.
Please review the following documents and make the appropriate contacts to obtain the documentation we are requesting. We encourage you to be prepared with this information as soon as possible. The Health Profession Immunization and History Form (below) will need to be received by the Health Center at Pacific University by July 16, 2008, or send this information in as early as possible during the summer.
