Instructions:
- Click on the underlined links below to download the required application forms. After downloading, save each form to your computer. After saving, users will be able to fill in responses, save those responses for a later time, and print the form out with it the responses included.
- Items 5 & 6 are not downloadable forms. Please follow the instructions below for these items.
- Please read the instructions and checklists before filling out the application material.
- All forms and materials must be handed in on paper with any required original signatures. We can't accept any electronic versions.
- Return all materials to the Pacific University International Programs Office, UC Box A153
The application form includes a section titled: "Personal Statement to Support your Application." You may choose to write your essay here, or you may attach it as a separate document (please see below under "Personal Essay" for further explanation).
- Office of Student Life Endorsement for Study Abroad
Fill in the information requested in the box at the top of the form before submitting it to the Student Life office.
- Two (2) Academic Recommendation Forms
Please read and complete the box at the top of each form before giving it to the faculty member who will write the recommendation. Please have these completed by instructors who are familiar with your work.
- Official Transcript/s
You must submit official college transcripts from Pacific University as well as all other colleges attended. Your transcripts must show credits and grades for all courses taken.
- Personal Essay
Please submit an essay about the life experiences that led to your decision to study abroad. Explain how you expect your experience abroad to fit into your college plans, your professional goals, and your personal goals. Limit your essay to two pages typed, double-spaced.
Please note that Pacific assesses a $250 processing fee per semester of study abroad that will be automatically included on your tuition statement each semester.
