The Graduate Certificate in Gerontology Program presents a 3-hour workshop for 3CE: Understanding Capacity and How it Affects the Discharge Process


PROGRAM GOAL  

Delayed discharge is not just a national imperative. It affects quality of life for patients, clients, and caregivers in terms of recovery and retaining highest level of independence.  This workshop focuses on how healthcare professionals determine functional and mental capacity.  It discusses how this information becomes criteria for discharge planning.  The workshop continues with case studies that examine discharge pathways for complex patients.  The final session focuses on integrated partners including systems within the healthcare process to reduce unnecessary delays, and looking beyond discharge at re-enablement to reduce readmissions.
