Student Placement Request Information - Eugene Campus
There are two documents that you will need to fill out in order for the process of placement to begin. Both these documents will need to be downloaded to your computer desktop, filled out, and then sent as attachments to Shelley Smolnisky, smolniskys@pacificu.edu.

Teacher Licensure Candidate
Placement Information - One of the requested items on this
sheet is contact information. Please fill out as much information as
possible. If we need to contact you and clarify the information we've
been given or to ask you a question about a school site, we need to be
able to get in touch with you. In addition, this document asks you to
tell us about your authorization level and, if secondary, your subject
area. Without this information we can't proceed with your
placement. Finally, we are asking you to indicate to us school district
preferences, if you have them. If you leave that section blank, we will
assume that you have no preference. The last section asks you to tell
us if there are some other considerations of which we need to be aware.

teach in a school near a bus stop? |
Teacher Licensure Candidate
Resume Information - This document will be sent to districts as
a way to introduce you to administrators and potential mentors. Again,
you are being asked to fill out contact information. Please make sure
that it is complete and accurate. The Type
of Placement section should be left blank; placement
coordinators on each campus will fill this out depending on the
experience requested. Please fill out the rest of the page as
thoroughly as possible. It's certainly okay to leave blanks if you
didn't, for example, receive any high school honors and/or activities.
Again, this will be sent out to school administrators, so you want to
make sure that you have no spelling and grammatical mistakes. The
placement coordinators will not be checking for that before we send it
out. If there is a glaring problem, we will return it to you to be
corrected. There are two other things of which to be aware. First,
there is a place to attach a photograph of yourself. Remember that this
is a professional document, so be sure that the photograph you attach
reflects that professionalism. If you don't have a photograph to
attach, we will attach the one we took for your student identification
card. Finally, the last page of the document is simply information for
building personnel to help them make a decision about whether they want
to be involved. |
