Admitted students must complete the health and required immunization form for your academic area and return forms to the Student Health Center prior to registration.
Required Immunization & Health History Forms
- Required Immunization and Health History Form for Undergraduates, Forest Grove College of Education and School of Professional Psychology students (PDF) This form to be used for students enrolled in the College of Arts and Sciences/Undergraduates, Forest Grove Campus College of Education (MAT/FLEX program), and Graduate School of Professional Psychology.

- Health Profession Student Required Immunization and History Form(PDF) This form to be used for students in Dental Health Science, Occupational Therapy, Optometry, Pharmacy, Physician Assistant, and Physical Therapy Programs. Immunization requirements (including proof of vaccination) must be on file BEFORE Registration and the start of your program. Some immunizations take 6 months to complete so please plan accordingly.
