Life Insurance, Long Term Disability and Accidental Death & Dismemberment Coverage
The University provides life insurance, long term disability and accidental death and dismemberment insurance coverage to benefit eligible employees starting on the first of the month following employment.
The University provides life insurance to benefit eligible employees starting on the first of the month following employment. The amount of life insurance is equal to one and one half times the annual salary (but not more than $200,000) earned by the employee and is payable to the beneficiary designated by the employee. Supplemental Life Insurance for the employee and dependents is available on an employee funded basis through payroll deduction.
Regence Life & Health is our life insurance vendor since 04/01/2011.
Employees may elect up to $150,000 in supplemental life coverage for themselves and $20,000 for their spouse, without having to provide evidence of insurability during new hire election period. If you had previously elected less than $150,000 for yourself, or $20,000 for your spouse you may elect up to those limits at a later time but you will have to provide evidence of insurability. Your current level of election will be included in your open enrollment materials which should be sent via email on Monday, Feb 13 2012.
New Travel Insurance Coverage
Regence Life also provides MEDEX Plus, a comprehensive program providing you with 24/7 emergency medical, security, and travel assistance - including emergency medical evacuation and repatriation - when you are 100 or more miles away from your permanent residence in your home country. (Expatriates are eligible regardless of distance from your expatriate home.)

Accidental Death & Dismemberment (AD&D)
Benefit eligible faculty and staff are included in the University's accidental death and dismemberment insurance on the 1st of the month following employment. AD&D coverage provides benefits when an employee suffers accidental bodily injuries which result in death or dismemberment.
Long Term Disability (LTD)
Following 180 days of short-term disability, the insurance carrier in accordance with the Group Policy provided by the University will provide long-term disability benefits. Premiums are paid by the University and all benefits received are taxable. Applications are available on our Forms web page and are mailed to the carrier for approval. After approval, the employee will receive the following:
- 60% of their regular monthly salary
- The University's contribution and employee contribution to the employee's designated 403(b) plan (up to 12%)
