Submitting Theses, Dissertations or Graduate Projects

School of Occupational Therapy
Critically Appraised Topics (CATs)
After your CAT has been approved by your professor, please follow these steps to submit:
- Complete (either electronically, or complete it by hand and scan it) this license agreement, which grants the Library/University the right to post your CAT online: OT CAT License Agreement (.doc).
- Email an electronic copy of your CAT (.doc or .docx format) to gilmani@pacificu.edu, along with the completed license agreement.
Innovative Practice Projects (IPP)
After your IPP has been approved by your professor, please follow these steps to submit:
- Complete (either electronically, or complete it by hand and scan it) this license agreement, which grants the Library/University the right to post your IPP online: OT Student License Agreement (.doc).
- Have an authorized representative from your practice site complete (either electronically, or complete it by hand and scan it) this approval form: Site Posting Agreement (.doc).
- Email an electronic copy of your IPP (.doc or .docx format) to gilmani@pacificu.edu, along with the completed license agreement and site approval form.
School of Physician Assistant Studies
Graduate/Capstone Projects
After your final paper has been approved by your professor, please follow these steps to submit:
- Go to the PA page in CommonKnowledge.
- Click on "Submit to CommonKnowledge" in the left menu bar.
- Create a new account in CommonKnowledge (this is different from your Pacific University ID and log-in).
- After your account is created, go back to the PA page and click on "Submit to CommonKnowledge" again.
- Read and accept the submission agreement.
- Fill out the information about your project and upload your paper.

Supplementary/Optional Forms: ALL Schools
In addition to the required license agreement form, you may find it desirable/necessary to use these additional forms:

| Creative Commons License (.doc) | If you would like to apply a Creative Commons license to your work, please submit this form in addition to the other required materials. |
| Embargo Request (.doc) | If you need to request that the posting of your paper online be delayed, please submit this form in addition to the other required materials. NOTE: If, for personal safety reasons, your work should NOT be posted online at all, please do not use this form; instead, email gilmani@pacificu.edu and copy/CC your faculty advisor on the email. |
