Release Form for Staff and Faculty Letters of Recommendation

It's that time of year when students ask for letters of recommendation. Staff and faculty must have students sign a release form when a letter of recommendation includes personally identifiable academic record information. Release form link listed below.

Staff and faculty are required to obtain a release form signed by a student before providing any personally identifiable information about that student when answering an inquiry. If no other signed release form is provided, this form must be used. Link to faculty and staff release form is now available on the Human Resources website under forms at 

Be sure to retain signed forms for your records.
