Re-applicants
Application Instructions
Submit a new online application which includes the following:
- Updated prerequisite worksheet and GPA calculation forms
- Updated pharmacy experience/observation hours table and write a reflection statement (within the application)
- Response to ALL of the Fall 2009 essay questions including the specific re-applicant question. The questions are new so you must provide new essay answers.
- $95 non-refundable application fee
Submit the following supplemental materials to the Office of Admissions
- Official transcripts for coursework completed since the last submitted application. Please indicate on your application if you want Pacific University to use any existing transcripts from your previous applications.
- Evaluation forms: You may use two new evaluators (one MUST be a pharmacist and the other may be an academic source) OR ask your previous evaluators to complete the NEW evaluation forms.
- Current resume or curriculum vitae (CV).
- TOEFL/IELTS Test Scores – if applicable.
