- Any student who has a university meal plan is not eligible for food stamps!
- If you live in university-owned housing, you must list your actual physical address when completing the form. If you live in Vandervelden Apartments, list your address as 2043 College Way , Forest Grove , OR 97116 , Vandervelden Apt. #_____

- Start out going South on College Way toward Pacific Ave/OR-8 W
- Turn Right onto Pacific Ave/OR-8 W.
- Turn Left onto Main St.
- Turn Left onto 19th Ave/OR-8E. Continue to follow OR-8 E.
