Food Stamp Information
On FG Campus

Department of Health Services staff will be on the Forest Grove Campus on Thursday, September 13 in the Multi-purpose Room (MPR) in the lower level of the University Center from 11-1pm to sign up students for food stamps. Students will be served on a first-come, first-served basis. Remember to have copies of all the documentation required (see the information to the left).

You must go to the
office and check in between 8-9am to be
able to apply that day. Be prepared to wait!!

PLEASE NOTE NEW ELIGIBILITY RULES:
Any student who has a university meal plan is not eligible for food stamps!
Information needed when signing up for food stamps:
- Photo ID
(can include Student ID, Driver's License or Passport ) - Verification of Social Security Number
( i.e. social security card, recent paystub or tax return) - Proof of Physical Address
( i.e. utility or phone bill for those not living on campus; for those in university-owned housing, may be obtained from the Student Housing Office in Student Life) - Financial Aid Award Letter
- Work Study Employment Verification
(the start date must be listed) - Monthly Rent and Utility Costs
(Special note to students living in University-owned Housing: Know your Monthly rent cost! This information can be obtained from the Student Housing Office in Student Life) - Verification of ANY income other than Federal Work Study, including any regular parental income
If you live in university-owned housing, you must list your actual physical address when completing the form. If you live in Vandervelden Apartments, list your address as 2043 College Way , Forest Grove , OR 97116 , Vandervelden Apt. #_____
Directions to Dept. of Human Services in Hillsboro :
- Start out going south on College Way toward Pacific Ave./OR-8 W
- Turn right onto Pacific Ave./OR-8 W
- Turn Left onto Main St .
- From 19 th Ave./OR-8 E, continue to follow OR-8 E
- Turn Left onto SE 10 th Ave./OR-8. Continue to follow SE 10 th Ave.
- Turn slight right onto NE Cornell Rd.
- Turn Right at 5920 NE Ray Circle
