University Technology Committee (UTC)

The University Technology Committee serves as a catalyst to explore the implications of new technologies as they impact the University community. It encourages exploration of topics of interest concerning how new technologies affect our educational mission, and how the University can best use promising innovations. Cell Phone Service Guidelines and Policy

The University policy on funding and use of employee cellular phones has changed as of April 12, 2010. Please review this policy before selecting one of the forms below for requesting University funded cellular service.

Cell Phone Stipend Agreement

The Cell Phone Stipend Agreement is for employees who spend considerable time outside of their assigned office area, or who must be accessible outside of scheduled or normal work hours, where a cellular or mobile device can be a significant benefit. Based on job duties, certain employees may qualify to be provided a stipend to cover the business use of personal cell phones.  This form must be completed and signed by a University VP prior to being submitted to the Technology Information Center for processing and delivery to Human Resources.

Department Provided Cellular Device Request Form

The Cell Phone Request form is for circumstances where a departmentally assigned cell phone may be provided by the University that is not assigned to a specific individual. In these cases, the University will provide cell phone service and equipment.   Please complete this form and have it signed by a University VP and submit this to the Technology Information Center for order processing.

Equipment Loan Agreement

This form is for everyone who checked out equipment from the Technology Information Center. This form must be completed each academic year.
