Where Can I Get Assessed?

This is a non-exhaustive list of resources for diagnosis and assessment. Please connect with OAA if you need assistance finding a provider.

Make sure the clinic you go to can provide the correct type of documentation. Please review our Disability Documentation Standards and Eligibility for Accommodations page before your visit to a clinic to ensure you obtain the documentation you need in the most timely and cost efficient manner possible.

Your Provider and Insurance - The easiest way to get documentation is by seeing your PCP or a provider you regularly visit. If you have a provider you saw for a while but stopped seeing you can usually contact their office for records.

Utilizing your insurance is a great way to get in contact with a provider if you don’t have one. You can call the number on the back of your insurance card or look up the website for your insurance carrier and search for providers. Usually, over the phone, someone can curate a list of local providers who take your insurance. In many cases, to see a specialist, you will need a referral from a PCP.

If you don’t have insurance you may qualify for Medicaid. The Oregon Health Plan provides medicaid in Oregon.

If you have insurance through your guardians and are afraid to ask them for your insurance card you can always tell them you need to see a doctor about a cold or call your insurance carrier and ask them to mail you a card. Due to HIPPA your insurance can’t divulge your personal medical information to your guardians.
