Faculty and Staff Flu Shot Clinic | Hillsboro
Kaiser Permanente-sponsored flu shot clinics will be onsite at the Hillsboro and Forest Grove campuses in October. Flu shot clinics are open to faculty and staff employees and their covered family members, who are nine-years-old and older. Children under 18 must have parental consent. Employees do not have to be Kaiser insurance members in order to participate.
Printing and completing an intake form prior to your flu shot clinic visit will save time.
Please also review the Vaccination Information Statement (VIS). Bring your insurance card to the flu shot clinic for review. Visit the Employee Wellness webpage to gain access to accepted insurance carrier information, the intake form, and the VIS.
