Pacific University COVID-19 Booster Clinic for Students, Faculty and Staff

Pacific University will host a COVID-19 booster vaccine clinic for all students and employees in the University Center on the Forest Grove Campus. Students are encouraged to make arrangements with professors in advance to attend the clinic. 

Plan to arrive early.  There is a limited vaccine supply. The last vaccine is at 12:45 p.m. or when the vaccine runs out. A post-vaccine observation period is required.

Please complete this fillable consent form to speed up the process. The clinic will also provide forms to fill out. 

All eligible Pacific employees and students must demonstrate proof of a booster dose by Feb. 11.

• Anyone who received a Johnson & Johnson vaccine can receive a booster two months after their initial vaccination.
• Anyone who received a Moderna or Pfizer vaccine can receive a booster five months after their second dose.
• Pacific community members who are not eligible for boosters prior to the Feb. 11 deadline will be expected to obtain boosters as soon as they become eligible.

Student Health Center appointments will also be available from 10 a.m. to 1 p.m. Thursdays through February.
