At Pacific University, we are committed to fostering an inclusive, respectful, and supportive community. If you have experienced or witnessed protected class bias, discrimination, harassment, sexual misconduct, or related retaliation, we encourage you to share your concerns through the Campus Climate Report Form.

Emergency Situations: Do not use this form to report immediate safety concerns. If you are in immediate danger or need urgent help, call 911 to contact local police and emergency personnel. You may also contact Campus Public Safety 24/7 at 503-352-2230 for the Forest Grove Campus and 503-352-7207 for the Hillsboro Campus.

What is a Campus Climate Report?

Campus Climate Reports help the university understand and respond to incidents that negatively impact our community. These reports allow us to:

  • Provide support and care to impacted individuals.
  • Learn more about incidents of concern.
  • Take appropriate action in response.
  • Foster education, dialogue, and healing.

Anyone — students, faculty, or staff — can submit a report. You may do so anonymously, though this may limit our ability to follow up or fully respond.

What to Expect After You Submit a Report

Step 1: Report is submitted

After you complete the form, you’ll receive an automatic confirmation. This message includes information about what happens next and available support resources.

Step 2: Report is reviewed

The coordinator may follow up with the reporter, the impacted person, and/or others involved to learn more about the situation and to discuss options for support and response.

Step 3: A response is developed

The process coordinator collaborates with relevant campus offices to determine how best to respond. Responses are tailored to the nature of the report, the needs of those involved, and the impact on the broader community.

Potential Responses

Depending on the situation, the university's response may include:

If the concern involves a potential violation of university policy or law, the matter will be referred to one or more of the following, as appropriate:

  • The Title IX, Title VI, Title VII, or Section 504 processes for review and possible investigation.
  • The Student Conduct Office (for concerns involving students or student groups).
  • A professional standards process (for professional students).
  • Human Resources or the Provost’s Office (for concerns involving employees or contractors).

For situations in which the climate concern source is either unknown or not clearly a student or employee, the next steps will be determined by the campus climate report coordinator.

If the concern is not a policy violation but still impacts individuals or the community:

  • Referral to a staff member for care, support, and mentorship.
  • Facilitated conversations, education, or restorative practices.
  • Coaching for students, faculty, or staff to foster accountability and growth.
  • Recommendations to university leadership to help inform broader policy or community improvements.

Every report is different, and the team is committed to responding thoughtfully and respectfully.

Your Privacy and Rights

We will treat you with respect, sensitivity, and dignity throughout the process. Your information will be shared only with those who have a legitimate need to know.

The university prohibits retaliation against anyone who makes a report or participates in a related process. We also do not tolerate discrimination based on a person’s status as a victim of domestic violence, sexual assault, or stalking.
