Complaints about the Physical Therapy Program for Members of the Public
Any individual, group, or organization may file a complaint about the Physical Therapy Program regarding any aspect of departmental personnel or functions. Complaints that arise from clinical education sites, employers of graduates, and the public should follow the process described below.
Formal complaints should be first submitted to the Program Director. The complaint must clearly describe the specific nature of the complaint, provide relevant information to substantiate the complaint, and outline any action(s) sought by the complainant. Complaints should be sent to:
Program Director, Physical Therapy Program
Pacific University
222 SE 8th Avenue, Office 342
Hillsboro, OR 97219
The Program Director will discuss the complaint directly with the party involved within 10 business days. The Program Director will then determine if the complaint should be managed using an existing mechanism within the University. If the complaint requires alternative management, a response letter from the Program Director will be mailed to the complainant, and a copy will be maintained by the Program for a period of 5 years.
If the complaint is not resolved at the Program level, or if the complaint is against the Program Director, the complaint will be addressed directly by the Dean of the College of Health and Rehabilitation Sciences within 10 business days. If the issue is not resolved to the satisfaction of the complainant after review and response by the Dean, then the written complaint may be filed with the Office of the Provost and Vice President of Academic Affairs:
Office of the Provost
Pacific University
Marsh Hall, Room 307
Forest Grove, OR 97116
The Provost will discuss the complaint directly with the parties involved within 10 business days and acknowledge resolution of the complaint via a letter sent to the complainant, Dean, and Program Director.
Records of complaints about the Physical Therapy Program, including the nature of the complaint and the final letter of disposition of the complaint are maintained by the Program Director for a minimum of 5 years.
