The following items should be arranged to be uploaded to our online application system:

  • Online Vision Science Graduate Program Online Application  from
  • Curriculum vitae
  • Application essay
  • Two letters of recommendation and Applicant Evaluation Forms
    • At least one reference letter must be from a class instructor.
    • in addition to the letter, your recommenders will also need to fill the Applicant Evaluation Form. 
  • Application fee: US$40.00
    • waived for current students and alumni of the Pacific University College of Optometry
  • Official transcripts from all colleges and universities attended
    • Transcripts are waived for current students and alumni of the Pacific University College of Optometry.
    • Applicants with OD degree from other ACOE-accredited optometry programs should submit the official transcripts (with cumulative GPA of 3.0 or better) along with course catalog to be considered for credit transfer.
    • Applications with foreign (non-US or -Canadian) transcripts should arrange to have their official transcripts sent through an International Credentials Evaluation agency.
      • Contact the International Credentials Evaluation agency service as early as possible. The services may take several weeks to process your foreign transcript once it’s received.)
