Ask your advisor about your catalog year if you are unsure. Your advisor's written approval is needed before this form can be processed.
- Focal Study Declaration Form (fillable pdf)
- Download the form first, then complete and save to your computer. For students in the 2017 catalog year or prior, use this form to declare your Focal Studies or make changes to previously declared Focal Studies. Signatures are not required.
- Focal Study Substitution Form (fillable pdf)
- Download the form first. Use this form to request a Pacific course to be substituted into a Focal Study. This form can be signed by either your advisor or an Advising Center advisor.
- Independent Study Contract (on Registrar's website)
- Complete the full form and obtain all signatures before submitting to Director of Academic & Career Advising for final signature.
